On October 21, 2026, in New York City, a joint operation targeting alleged healthcare fraud was carried out by multiple law enforcement agencies, including the Federal Bureau of Investigation (FBI), Homeland Security Investigations (HSI), Internal Revenue Service Criminal Investigation Division (IRS-CI), and the New York City Police Department (NYPD). Federal agents were seen removing a large amount of documents and computers from adult day care centers in Flushing and Borough Park, Brooklyn.
According to a video released by FreedomNews.TV, law enforcement officials were present at 35-20 146th Street in Flushing. Personnel were seen carrying boxes and electronic equipment in and out of the building, including IRS criminal investigators and officers from the NYPD.
Official records from the New York City Department for the Aging show that 35-20 146th Street is home to a social adult day care center called Sunrise Senior Service LLC. The center is listed in the 2026 Long-Term Care Facility Directory, specializing in providing services in Korean language.
On the same day, Mehmet Oz, the director of the Centers for Medicare and Medicaid Services (CMS), personally visited two adult day care centers in Flushing to investigate alleged improper reporting of Medicaid expenses. Oz visited Kent Senior Care and Evergreen Adult Day Care, issuing warning letters to address concerns regarding their Medicaid reporting records.
During Oz’s visit to Kent Senior Care, questions were raised about the attendance records of elderly individuals and the reporting of wheelchair transportation services. The center’s management denied inflating attendance numbers, explaining that some seniors arrive late, and only actual service recipients are reported.
At Evergreen Adult Day Care, Oz inquired about daily attendance numbers, transportation services, and sign-in records of elderly individuals.
Video footage from FreedomNews.TV also showed that adult day care centers in Borough Park were searched on the same day. Federal agents conducted searches at Warm Home Adult Activity Center on Fort Hamilton Parkway and Joyful Adult Day Activity Center on 8th Avenue, seizing evidence.
The FBI stated that they coordinated with multiple law enforcement agencies in Queens, Brooklyn, Staten Island, and other locations in New York City for the federal investigation operation.
Governor Hochu of New York noted that the investigation targeted social adult day care service providers suspected of money laundering, kickbacks, improper billing practices, and awaits judicial proceedings to determine the validity of the allegations.
Social adult day care centers primarily offer daytime activities, socialization, meals, supervision, and personal care services to eligible seniors and adults in need of care. Some service costs are covered through the Medicaid Managed Long-Term Care Program.
In February of this year, the New York State Comptroller’s Office released an audit report on social adult day care centers, highlighting issues such as lack of sufficient evidence for Medicaid payments, incomplete attendance records, and overreporting of attendance numbers beyond facility capacity.
The audit revealed that some centers lacked proper documentation to support their service claims, underscoring deficiencies in the state’s oversight of these facilities.
